Tammy brings 30+ years experience in nursing, and over 25 years as a triage nurse. Tammy is an absolutely wonderful advocate for our patients -- she is available to help answer your real-time healthcare-related questions over the phone. She assists with coordination of urgent care needs, timely prescription refill requests, and prior authorizations. Tammy also leads both our Chronic Care Management program and Transitional Care Management facilitating smooth transitions for our patients who are seen in the ER or hospitalized, making sure these patients details of transitions are all in place working closely with our clinic providers!
